Explanation: Mi-Forms Server installs a service called "Mi-Forms Server Monitor" which by default accesses the default customer (CID="") so that IIS will re-initialize the Mi-Forms Server web application if the web.config file gets updated during configuration and use of the Mi-Forms Server.
Solution: Configure Mi-Forms Server Monitor or remove the name of one of the customer accounts. To configure Mi-Forms Server Monitor, open "MFServerMonitor.conf" (typically found in c:\Program Files\Mi-Co\Mi-Forms Server 6\) and edit the following line:

by inserting the name of a customer into the value, ie:
